Athletic Medical Eligibility Forms Available With New NCAA Info
Each year, every Eastern Mennonite University athlete must fill out a medical eligibility form. The 2010-11 forms, which are now available on the EMU Athletics website, also feature new NCAA initiatives in health and safety. The new forms include statements regarding concussion management, sickle cell trait and also a new section for identifying medications that require further medical documentation prior to possible NCAA drug testing.

All EMU athletes need to have current Medical Eligibility and Pre-participation Physical Exam documents on file with the Athletic Training Department one week prior to any participation in formal team practices. (Fall sport deadline is Mon., August 16.) These forms can be found at www.emu.edu/athletics/at/medical_forms.

Physical exams must be obtained after July 1 to be considered current for the upcoming year. All first year and transfer athletes will also need the EMU Health Services' Health Evaluation and Immunizations record to be approved and on file with the University Health Center to be considered for athletic medical eligibility.
