Print & mail forms: Letter of Recommendation**, Transcript Request**

Step 3:
Complete on-line graduate application form (M.A. or Graduate Certficate)
   
Step 4:
Mail your check in the amount of $25.00 to cover your application processing fee along with signature page
   
Step 5:
 Send remaining documentation to complete your application file
