REQUESTING A TRANSCRIPT

Academic transcripts may be requested by filling out a Transcript Request Form (PDF) at the University Registrar’s Office or by sending a written request, including the signature of the person whose transcript is requested, to:

Information you need to provide: If you are writing from off-campus, be sure to include your birth date or social security number and full name to help us identify the proper record. If your name has changed, include both your present name as well as your name when you last attended. It’s a good idea to include a daytime telephone number in case we need to contact you for any reason.

Faxed transcript requests: Transcript requests by fax will be accepted as long as the request includes the student’s signature. Email requests will be accepted if the request form is signed, scanned, and attached in an email.

Processing time: While we do our best to provide transcripts promptly, we ask you to allow up to a week’s turn-around time.

Accounts cleared through Student Accounts Office: All transcript requests must be approved by the Student Accounts Office, to assure that there are no outstanding financial obligations to EMU. Contact Student Accounts at 1-888-432-4112 if you have questions about whether your account is clear.

Fees: Payment of $5.00 per copy is required prior to processing. Please include a check payable to EMU with any request submitted by mail. If you choose to fax your request, you will need to make payment using a credit or debit card. You can make payment or by contacting Student Accounts at 1-888-432-4112.

Mailing transcripts: We will send a transcript to you or to whatever other address you provide. If you are applying to graduate schools, read your instructions carefully to be sure you comply with each school’s application procedures.

Faxing transcripts: We will send your transcript by fax if you include instructions in your written request. Keep in mind, however, that EMU cannot assure the confidentiality of your transcript at the receiving fax machine, and that faxed transcripts are not considered official because they do not contain the university seal.
