Requesting Immunization Records

From EMU:
Complete EMU’s Authorization for Use or Disclosure of Health Information form and send it through campus mail to: Vanessa Derrow, Health Center (or take it to the Wellness Suite)

Instructions:

• #2) Check “Immunizations”, and for “Dates” write: “ALL”

Contact your doctor’s office right away (sometimes they take a long time to complete these requests). They might have a specific form for you to fill out, but usually they just ask for something in writing with your date of birth and signature giving express permission for me to receive your records.

Note: We have your records sent to us instead of directly sending them to the university you’re applying to because sometimes the universities lose them and it’s faster for us to make another copy.
