Complete two forms to apply to IEP:

1. Most students: IEP Online Application OR IEP Application Form (PDF; send as email attachment or print and mail)

Students transferring from another U.S. college or university: Transfer Form (PDF; send as email attachment or print and mail)

2. All students: IEP Financial Certification Form (PDF; send as email attachment or print and mail)

When finished, send PDF forms to:
