Homecoming Registration Form

Please fill out this form and send payment to the Alumni Office by September 28 2008. Payment for homecoming can be sent through the mail via check or on the web via credit card. List only the names of those attending and indicate how the names should appear on the name tags. Please complete this form even if you do not require tickets to any events. It will allow you to pick up a registration packet with a name tag, schedule of events and other goodies when you arrive. Please include birth name.
