Athletic Medical Eligibility Forms & Pre-Participation Physical Exam
The link below will provide the instructions and forms necessary to be medically eligible to participate in intercollegiate athletics at EMU for the 2011-12 academic year. ALL ATHLETES PLEASE NOTE:
Be aware that there are two medical documents you need to complete and have on file as an EMU student-athlete. The first form for athletes is the Athletic Medical Eligibility Packet found at this site. All Fall sport athletes must complete and return the packet by mail to the address listed by Monday, August 15, 2011. Athletes on all other teams must complete and return the Athletic Medical Eligibilty Forms one week prior to the first practice session of their season. This includes sports which have non-traditional seasons.

The second medical document is for transfer and first year students only. This is the EMU Student Health Evaluation Form issued by the University Health Center. If you did not receive this with your original admissions packet, click here and go to the Student Health Evaluation Form. While in most cases this form does not require a physical exam, it is required by the University as it includes your immunization records.

Athletes are required to repeat the Athletic Medical Eligibility Forms each year whereas the EMU Student Health Evaluation Form is only completed upon enrollment. Returning athletes do not need to do this if they have previously returned the EMU Student Health Evaluation Form. First year and transfer athletes please complete and return both sets of forms. The two documents are not the same, and one will not be accepted for the other.
