Reporting Accidents at EMU

We ask anyone who is injured in an accident while on the campus of EMU to report the incident to the Human Resources office by completing this Employee_Accident_Form and submitting it to hr@emu.edu

Non-Employees should complete and return Non-Employee_Accident_Form
