YOU ARE RESPONSIBLE to meet all deadlines!
Send $150 application fee to LEAP Office to confirm your spot in the program.
Be sure you have a working email address and that you have given it to the LEAP Office so you’ll receive communications.
Please send us a clear photograph of yourself; if possible, from shoulders up.

Work to earn money to pay for your LEAP tuition and your spending money while you are on cross-cultural. GOAL: $1,350 or more in 4 months. That’s $85 a week.
Have your parent(s) or guardian(s) sign ALL attached permission forms and return them to the LEAP Office.
Attach proof of your insurance coverage while abroad to the family medical form.
Send a photocopy of your birth certificate to the LEAP365 Office.
Return Scholarship Application to LEAP Office if you have financial need.
Have your passport photo taken. Buy EXTRA COPIES – at least four in all.
Apply for a passport & any necessary visas WITH EXPEDITED SERVICE.
Make an appointment with a doctor for a physical exam, immunizations and TB tests.
Send first $450 payment to LEAP Office. (Non-payment will drop you from the program.)
Send the enclosed form showing proof of your physical examination INCLUDING photocopies showing proof of your immunizations and TB test to the LEAP Office.
Call the LEAP Office if you have NOT yet received your passport/visas. (540.432.4698) If you HAVE your passport, send in a photocopy.
Send second $450 payment to LEAP Office. (Non-payment will drop you from the program.)
Arrange for your own transportation TO LEAP in Harrisonburg Virginia on July 8 and FROM LEAP when the program ends on July 25, 2009. Send notification of your plans to the LEAP office by email or regular mail.
Purchase appropriate bathing suit and other clothing for your cross-cultural.

Bring final $450 payment to LEAP Registration. If you are awarded a scholarship, it will pay for this final payment only. Your account must be PAID IN FULL on arrival.
DETAILED INFORMATION ABOUT THE CHECKLIST
LEAP Payments -Due May 1, June 1, and July 8
After you send in your $150 application fee, there is still $1,350 due to LEAP to pay for your meals, lodging, transportation on the ground and airline tickets for the three-week program. How do you pay for that?
If your family cannot afford the whole amount, consider this:
Call the LEAP Office in advance if you are unable to make a $450 payment by the dates on the checklist. Otherwise you will be dropped from the program . All payments must be complete by the arrival date of July 8, 2007.
Passport and any necessary visas -Due May 1
Passport and/or visa applications should be made immediately in person and/or by mail. Pay for EXPEDITED service. Last year two students were unable to leave with the cross-cultural group because their passports did not arrive in time. There is information about passport applications at www.usps.com/passport/welcome.htm. You could also check with a local post office or a town/city/county clerk’s office about how and where to apply for a passport.
It takes about SIX WEEKS (more or less) for the government to process an application and send the approved passport to the applicant. The government may also send back an application that is incomplete or needs other clarification, causing a longer delay. Be sure to fill out every application correctly, and include all the necessary information and requested documents. Passport applications require two "passport-size" photos to be sent along with an application. Please purchase at least four passport photos and send the extras to the LEAP Office. Local stores, camera shops, and photographers can take these for you.
NOTE : Permanent U.S. resident aliens who possess a green card will need a valid passport from their country of national origin and any necessary visas for the countries to which we will be traveling.
Insurance Coverage -Due April 1
You are responsible for insurance coverage for yourself while you are abroad. The LEAP Office must have a photocopy showing proof of your international coverage. Include a photocopy of both sides of your insurance card and medication card (if any).
Physical Checkup, Immunizations and TB test -Due May 1
Talk to your doctor and/or your local Department of Health about where/when you can get the immunizations and the costs involved. Often the department of health is less expensive if you do not have insurance coverage.
Some of the immunizations need several shots a month or two apart, so make an appointment immediately ! By May 1 send the physicians medical form showing that you have had a checkup, and a photocopy of all of your immunizations since birth, including the ones below. PLEASE KEEP AN ACCURATE RECORD OF THE IMMUNIZATIONS YOU RECEIVE.
The following are recommended by the Eastern Mennonite University Health Center. You must send proof by photocopy that you have had these. If your doctor recommends something different, please call us.

3. Arrange for your own transportation : 1) from your home to Eastern Mennonite Seminary's campus in Harrisonburg, Va. 2) back to your home from Eastern Mennonite Seminary at the conclusion of the LEAP 2009 Summer Experience. We would encourage you to seek out any possibility for cooperative travel between you and other LEAP participants who hail from nearby areas. We will send you a complete list of participants later so that you know who is coming. It may be convenient to combine your travel resources and efforts with others.
Please inform the LEAP office in writing by email or regular mail of your specific travel plans for arriving and departing for the LEAP 2009 Summer Experience.
4. Check the LEAP website www.leap.emu.edu frequently for updates and important information. Have your family and friends check it frequently, as well. The web site and email will be the primary line of communication between you, LEAP, and your family and friends throughout your entire LEAP experience.
5. Check, double-check, and keep checking to make sure you have read, understood, and completed all of the information, instructions, and requests we have given you via mail, email, telephone, or the web site. These preparations are vital. You are responsible to make all necessary preparations for your trip.
